Ambulatory Living

In the Ambulatory Living of the Martinsclub Bremen e.V. we support people with mental disabilities in their own appartments after they successfully participtated in the m|c Resident Training.

The m|c concludes a care contract with the users in which are inter alia enlisted necessary aids and aims like support in the household, the finances, official concerns, accomodation concerns, visitation of practitioners oder crisis intervention.

Those measures are implemented by the common and close employees of the m|c Resident Training. That assures a high consistency of the care and an artificial break caused by the move into the Assisted Living is avoided.

In case of emergency the users can contact the team of the m|c Resident Training. We also offer thematical common activities in joint with the users of the m|c Resident Training.

Other support services like home help, neighbourhood help oder else up to ambulant nursing can be organized by the ambulantory care.

Integration into the districts is initiated by the moderation of relationships to neighbours and local services. The aim is further independence up to living utterly self-determined. The value of people with mental disabilities for the togetherness in the neighbour is thus being mediated.
